Background: Thrombotic thrombocytopenic purpura (TTP) is a rare but life-threatening thrombotic Microangiopathy characterized by severe thrombocytopenia, Microangiopathy hemolytic anemia, and microvascular platelet-rich thrombosis resulting from critical deficiency of ADAMTS13. Immune-mediated TTP (iTTP) arises from autoantibody inhibition of ADAMTS13, whereas congenital TTP (cTTP) results from inherited mutations. Rapid recognition is essential, as untreated disease carries high mortality. Recent Advances (2020–2025): The past five years have marked significant progress in understanding and managing TTP. Improved insight into ADAMTS13 conformation, immune dysregulation, endothelial activation, and inflammatory pathways has refined disease classification and risk assessment. Diagnostic strategies increasingly incorporate rapid ADAMTS13 assays and validated clinical prediction tools such as the PLASMIC score, facilitating early therapeutic intervention. The introduction of Caplacizumab, a nanobody targeting the A1 domain of von Willebrand factor, has transformed acute management by accelerating platelet recovery and reducing early complications. Rituximab is now widely used as early adjunctive therapy and for relapse prevention. In congenital TTP, recombinant ADAMTS13 represents a promising step toward physiologic enzyme replacement, reducing reliance on plasma infusion.Clinical Implications: Contemporary management emphasizes immediate initiation of plasma exchange, integration of targeted therapy, and risk-adapted immunosuppression. Monitoring ADAMTS13 activity during remission has improved relapse prediction and enabled preemptive treatment strategies. Increasing recognition of long-term neurocognitive and cardiovascular sequelae underscores the need for structured survivorship care. Conclusion: Between 2020 and 2025, TTP management has evolved toward precision-based, mechanism-directed therapy. Continued efforts to expand diagnostic access, optimize individualized treatment, and address global disparities are essential to further improve outcomes for patients with this complex disorder.
